In anticipation of being accepted, please email (referral@tlwtransformationcenter.com) the following documents as they will be required for the Welcome Visit.

Educational

- Most recent report card
- Most recent ARD/IEP
- School withdrawal form
- CMBHS CANS
- Any school incident reports

Psychological

- Last 90 days of the psychiatric notes and medication monitoring notes
- Last 6 months of hospitalization info (if any)

Mental Health

- Last 90 days of individual/group/family notes

Clinical

- DFPS Child and Family Service Plan
